- business located in the United States
- sales channel, retail store, and/or web site related to health and well-being
- tax exemption certificate. Without that, we are legally obligated to charge tax on applicable items. The name on the certificate should match the company name associated with your account.
PLEASE NOTE: Applications will be reviewed within 5 business days and determination will be made at that time.
You will first need to create an initial D'Adamo Personalized Nutrition login and then will be automatically redirected to sign a Minimum Advertised Pricing (MAP) agreement and upload any necessary documents.
